Pet care agreement and release
I am the owner, or I am authorized by the owner, of the pet or pets I list below, and I agree to the following with [Your business].
Care
- My pet is in good health and up to date on required vaccinations, and I will show proof if asked.
- Play with other animals carries risks, including scratches, bites, illness and injury, even with careful supervision.
- I have told [Your business] about any aggression, medical condition or special need.
- Grooming can reveal or worsen skin, coat or health problems that already existed.
Veterinary care
If my pet needs urgent care and I cannot be reached, I authorize [Your business] to take my pet to a veterinarian, and I agree to pay the costs.
Release
To the fullest extent allowed by law, I release [Your business], its owners and staff from claims arising from the care of my pet, except those caused by gross negligence or willful misconduct. I agree to pay for any damage or injury my pet causes to people, animals or property.
General
This agreement is governed by the laws of the place where [Your business] operates. If any part is found invalid, the rest stays in force. I have read this agreement and I am signing it voluntarily.
