Tattoo consent form template

I am asking [Your business] to give me a tattoo or piercing. I agree to the following.

My consent

• I am at least 18 years old, and I will show valid photo ID.
• I am not under the influence of alcohol or drugs.
• I have approved the design, spelling and placement. Once applied, a tattoo is permanent and can only be removed with further treatment at my own cost.
• I understand that colors and lines may vary, and that results depend on my skin and how I care for it.

Health and aftercare

• I have told the artist about any medical condition, allergy, medication, pregnancy or skin condition that could affect the procedure or healing.
• I understand there is a risk of infection, allergic reaction and scarring, and that following the aftercare instructions reduces it.
• I will follow the aftercare instructions I have been given, and see a doctor if I notice signs of infection.

Assumption of risk

I understand that tattooing and body piercing, including pain, infection, allergic reaction and scarring involves risks, including the risk of serious injury, permanent disability and death, and damage to or loss of property. I choose to take part voluntarily and I accept these risks, whether they are known or unknown.

Release of liability

To the fullest extent allowed by law, I release, waive and discharge [Your business], its owners, employees, instructors, contractors and agents (together, "the Released Parties") from any and all claims, demands or causes of action arising from my participation, including those caused by the ordinary negligence of the Released Parties.

Indemnity

I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage or cost, including legal fees, that arises from my participation or my conduct.

Medical treatment

I confirm that I am in good health and have no condition that makes it unsafe for me to take part. If I am injured or become ill, I authorize [Your business] to arrange first aid or emergency medical treatment, and I agree to pay any costs.

Photos

I agree that [Your business] may take photos or video during activities and use them to promote its services, unless I tell staff that I do not agree.

General

This agreement is governed by the laws of the place where [Your business] operates. If any part of it is found to be invalid, the rest stays in force. I have read this agreement, I understand it, and I am signing it voluntarily. I understand that I am giving up substantial legal rights by signing.
