Your business name

Jump and play waiver

This waiver applies to every visit to [Your business], including trampolines, foam pits, climbing walls, soft play and parties.

Park rules

  • One person per trampoline. No flips or somersaults unless a staff member allows them.
  • Grip socks must be worn. Empty your pockets before jumping.
  • Children under 7 must be supervised by an adult at all times.
  • Staff may ask anyone who breaks the rules to leave, without a refund.

Assumption of risk

I understand that jumping, climbing and play activities, including collisions, falls and landing awkwardly involves risks, including the risk of serious injury, permanent disability and death, and damage to or loss of property. I choose to take part voluntarily and I accept these risks, whether they are known or unknown.

Release of liability

To the fullest extent allowed by law, I release, waive and discharge [Your business], its owners, employees, instructors, contractors and agents (together, "the Released Parties") from any and all claims, demands or causes of action arising from my participation, including those caused by the ordinary negligence of the Released Parties.

Indemnity

I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage or cost, including legal fees, that arises from my participation or my conduct.

Medical treatment

I confirm that I am in good health and have no condition that makes it unsafe for me to take part. If I am injured or become ill, I authorize [Your business] to arrange first aid or emergency medical treatment, and I agree to pay any costs.

Photos

I agree that [Your business] may take photos or video during activities and use them to promote its services, unless I tell staff that I do not agree.

General

This agreement is governed by the laws of the place where [Your business] operates. If any part of it is found to be invalid, the rest stays in force. I have read this agreement, I understand it, and I am signing it voluntarily. I understand that I am giving up substantial legal rights by signing.

Minors

If I am signing for a child under 18, I confirm that I am their parent or legal guardian, that I agree to these terms on their behalf and on my own, and that I have explained the rules and risks to them.

The signer ticks each of these

  • I have read the park rules and explained them to the children I am signing for.
  • I will supervise children under 7 at all times.
